Due to the CDC and local guidelines, masks are required before entering our offices. In addition to limit the spread of COVID-19, we are only allowing one person per appointment to enter our office (including the waiting room). We realize this can be challenging for family members and friends who would like to accompany you to our office, but know it is with your health and the health of our staff at our highest priority that we make this decision.
NOTE: If you or a close contact (someone you have spent over 15 minutes with less than 6ft away) have tested positive for COVID-19, please call our offices to reschedule your appointment and for any further information.
You will now receive a voicemail or text reminding you of your appointment the day before you are expected at our office! If you are unsure of when your appointment is scheduled, feel free to give us a call at either office location.
The location of our offices and where your appointment will be should be in your text and voicemail reminder. If it is not included, or you are unsure, feel free to call us and ask!
Please make sure to have your insurance card and account information with you on your visit! Double check to see if we take your insurance provider under the tab "Insurances" above. Unfortunately, we do not take the following insurances: Fidelis, Healthfirst, Metroplus, or Medicare Primary; however, we can offer you a discount for your appointment.
